Medical Disclaimer for Online Pre-evaluation & Patient Candidate Obligations
This document outlines the mandatory requirements for all patient candidates during the pre-evaluation stage. Transparency and accuracy are essential to ensure your safety and the success of your surgical planning. By proceeding with your pre-evaluation, you acknowledge and accept the following terms.
1. Obligation of Accuracy and Disclosure
Patient candidates are required to provide complete, accurate, and honest medical information during all stages of evaluation. This includes, but is not limited to:
- Medical History: All pre-existing conditions, previous surgeries, and prior medical procedures.
- Medications and Treatments: Current medications, including any hormone therapies, antibiotics, or supplements.
- Substance Use: Full disclosure of smoking or nicotine use (including vaping/e-cigarettes), alcohol consumption, and any history of drug use or addictions.
- Health Status: Recent or current medical issues (e.g., infections, coagulopathy/bleeding disorders, hypertension, or diabetes).
2. Clinical Photography Requirements
Clinical photographs are fundamental to your surgical plan. Patient candidates must provide high-quality, clear images as requested by the clinic. Providing incomplete or low-quality documentation may result in an inaccurate surgical assessment, which can lead to complications or the inability to proceed with the planned surgery.
3. Consequences of Non-Compliance
Failure to disclose relevant medical information or provide adequate clinical documentation can lead to significant outcomes, including:
- Surgical Risk: Hiding medical details (such as blood coagulation issues, drug use, or previous surgeries) may lead to life-threatening complications or suboptimal results.
- Cancellation: Dr. Sürmeli reserves the right to postpone or cancel the operation if the patient candidate is found to be non-compliant, if health risks exceed international safety limits, or if there is a concern regarding the patient candidate’s willingness to follow pre-operative instructions.
- Financial Impact: Cancellation of surgery due to non-compliance or undisclosed health issues may result in the forfeiture of deposits and the loss of any related travel or accommodation costs.
4. Legal Acknowledgment & Consent
By proceeding with the pre-evaluation and pressing “Next,” and by submitting their medical information and clinical photographs, the patient candidate confirms that all information provided is accurate and complete to the best of their knowledge. They understand that their safety and surgical outcome depend on their full transparency and adherence to these clinical requirements.
Please note that an online pre-evaluation does not establish a formal patient-doctor relationship. A formal medico-legal patient-doctor relationship commences only after a face-to-face consultation and mutual agreement between both parties regarding the treatment plan and options.
