The technique matters: dual-plane placement
Where the implant sits affects how your breast looks, feels, and ages. Our standard approach is the dual-plane technique: the upper part of the implant is covered by the chest muscle (a soft, natural transition — no visible implant edge), while the lower part sits behind the breast tissue itself (a natural lower curve). Compared with simple over-the-muscle placement, this gives a more natural upper breast, a lower rate of capsular contracture (hardening), and less visible rippling — especially in slim patients.
The incision is small and hidden in the natural fold beneath the breast, where it typically fades to a barely noticeable line.
An honest word about fat transfer
Some clinics promote breast enlargement with your own fat as an equal alternative to implants. Dr. Sürmeli does not present it that way, and you deserve the full reasoning. Fat grafting to the breast has real, well-documented limitations. A large and unpredictable portion of the injected fat — often anywhere from 40% to 80% — is reabsorbed by the body, and the amount that survives differs from one breast to the other, so the final size cannot be reliably planned or guaranteed. The fat that does not survive can lead to fat necrosis, oil cysts, and calcifications — and those calcifications can mimic the appearance of breast cancer on mammography, causing confusion during future screening. There is also an infection risk, because the breast’s milk ducts naturally harbour bacteria. And even at its best, a breast enlarged with fat alone often does not achieve the firm, full, natural consistency patients expect from augmentation. For all these reasons, in this practice fat grafting to the breast is reserved for fine-tuning minor asymmetries — usually alongside or after implant surgery — and is never a one-to-one substitute for breast augmentation.

Around months 2–3 the implants settle into their natural position — patients call it “dropping and fluffing.” The final shape is judged at 6–9 months, not before.

Implants do not increase breast cancer risk, and the implants used in this practice belong to the lowest-risk category for rare implant-associated conditions. Screening stays simple: tell your radiologist you have implants, and as a baseline we perform a breast ultrasound before surgery. Whenever a question arises — even years later — our team is easy to reach.
Over 18, in good general health, a non-smoker (or fully stopped at least 4 weeks before and after surgery — this is non-negotiable), with realistic expectations. If pregnancy is on your near horizon, surgery is usually planned after.
